About the role
The Patient Outreach Specialist conducts structured, telephonic wellness check-ins and assessments with patients on behalf of healthcare partners. This role allows healthcare graduates to gain clinical experience while helping organizations extend their capacity through evidence-informed patient interactions. Specialists work independently from a private, quiet workspace to support patients with various health needs.
Responsibilities
- Complete a minimum of 20 hours of patient outreach per week on a flexible schedule
- Conduct structured telephonic assessments and wellness check-ins using the company platform
- Document interaction outcomes accurately within the platform
- Escalate patient safety concerns to a licensed clinical supervisor in real time
- Maintain strict HIPAA compliance and patient confidentiality at all times
- Participate in onboarding training and ongoing check-ins
