About the role
The Patient Outreach Specialist conducts structured, telephonic interactions with patients on behalf of partner healthcare organizations. You will follow evidence-informed protocols to perform wellness check-ins, complete assessments, and escalate concerns while gaining clinical experience. This role requires working from a private, quiet space and maintaining strict HIPAA compliance.
Responsibilities
- Complete a minimum of 20 hours of patient outreach per week on a flexible schedule
- Conduct structured telephonic assessments and wellness check-ins using the company platform
- Document interaction outcomes accurately within the platform
- Escalate patient safety concerns to a licensed clinical supervisor in real time
- Maintain strict HIPAA compliance and patient confidentiality at all times
- Participate in onboarding training and ongoing check-ins
