Patient Outreach Specialist

About the role

The Patient Outreach Specialist conducts structured, telephonic interactions with patients on behalf of partner healthcare organizations. You will follow evidence-informed protocols to perform wellness check-ins, complete assessments, and escalate safety concerns while maintaining HIPAA compliance. This role is designed to provide healthcare graduates with real-world clinical experience in a remote setting.

Responsibilities

• Complete a minimum of 20 hours of patient outreach per week on a flexible schedule
• Conduct structured telephonic assessments and wellness check-ins using the company platform
• Document interaction outcomes accurately within the platform
• Escalate patient safety concerns to a licensed clinical supervisor in real time
• Maintain strict HIPAA compliance and patient confidentiality at all times
• Participate in onboarding training and ongoing check-ins
