Patient Outreach Specialist

About the role

As a Patient Outreach Specialist, you will conduct structured, phone-based interactions with patients on behalf of partner healthcare organizations. You will follow evidence-informed protocols to check in on patients, complete assessments, and escalate concerns to clinical supervisors. This role provides healthcare graduates with real-world clinical experience while helping healthcare organizations extend their capacity.

Responsibilities

• Complete a minimum of 20 hours of patient outreach per week on a flexible schedule
• Conduct structured telephonic assessments and wellness check-ins using the company platform
• Document interaction outcomes accurately within the platform
• Escalate patient safety concerns to a licensed clinical supervisor in real time
• Maintain strict HIPAA compliance and patient confidentiality at all times
• Participate in onboarding training and ongoing check-ins
